AB HIV infection in haemophilia differs in several respects from infection
in other groups. Co-infection with Hepatitis C (HCV) and/or Hepatitis B
(HBV) is common and its course may be modified by HIV. HIV may also
alter the natural history of haemophilia. This paper presents data on 75
evaluable persons with haemophilia infected with HIV between 1981 and
1985. 59 (79%) were alive and 16 (21%) had died (July 1993). Cumulative
incidence of AIDS was 23%. 9 (12%) had died of AIDS. No deaths were due
to liver failure despite universal co-infection with HCV. There was no
KS, CMV retinitis or toxoplasmosis despite sero-prevalence of 60% and
30% respectively. There were altered immune responses to HBV in 16% of
those co-infected with HIV/HBV. HIV exposes the risk of septic arthritis
in haemophilia and is associated with disappearance of allo Factor VIII
inhibitors. Because many haemophilia patients have chronic hepatitis it
is necessary to modify investigation and prescribing habits with regard
to AIDS management.
